Peer Support Booking Form

Peer Support Booking Form

Thanks so much for your interest in Peer Support! First, make sure you’ve clicked on your desired session & changed the text from AVAILABLE to BOOKED on the calendar. Then, fill out this intake form!

We just wanted to remind you that our peer supporters are fellow medical students, and so we are not a professional resource. If you’d like to seek out a professional resource, feel free to check out our resource sheets or talk to a peer supporter to find out what may be the best professional support for you! As well, know that peer supporters follow a strict confidentiality policy (check out the FAQ!)

*If you are in a crisis and there is an imminent risk of harm to yourself, to someone else, or to a minor (18 and under), please reach out to the following now, instead of waiting for an appointment. We want you to be safe as soon as possible!*

1-800-663-6729: href="https://www.physicianhealth.com/">Physician Health Program, available 24/7

604-872-3311 or https://crisiscentre.bc.ca/: Crisis Intervention & Suicide Prevention Centre of BC)

1-800-SUICIDE (1-800-784-2433): 24/7 provincial access for anyone thinking of or affected by suicide, or concerned about others’ safety

Crisiscentrechat.ca (age 25+) or youthinbc.com (<25): Online crisis support

9-1-1: This may be scary, but if you are in immediate danger, first responders are trained to support you, de-escalate, and help get you to safety at the nearest hospital.

I have acknowledged that I’ve read the blurb above and am not in a crisis(Required)

When you book a session, your peer supporter does not receive your e-mail address. We only collect this so that the Zoom link can be sent automatically to your secure UBC e-mail inbox. This info is only accessed by a human in case of emergencies (the 3 exceptions in FAQ #1), so that ourselves & emergency services have a way of reaching you to assure your safety. We need a UBC e-mail address in order to verify that you are a UBC medical student, again, only in emergency situations.
